Financial Options
CARECREDIT
We offer CareCredit, a healthcare credit card tailored for treatments and procedures not covered by insurance. Once approved, your entire family can use the same card. CareCredit offers two distinct features: every transaction qualifies for a No Interest If Paid in Full Within Promotional Period Payment Option or a Reduced Interest Payment Option. PREPAY
For estimated charges over $1000, a 5% prepay on the estimated patient portion of the treatment fee will be offered to those making payment in full by cash or check at least three days before scheduled treatment.
PAY AT TIME OF SERVICE
Payment is required in full for treatment provided during single appointments. For extended treatment procedures, the total cost may be divided by the expected number of appointments, with equal amounts due per appointment.
The office manager must arrange and agree upon this in advance.

We accept MasterCard, VISA, Discover and CareCredit.
DENTAL INSURANCE
Insurance coverage varies based on your policy and the premiums paid by your employer. It is one of several savings options available. Still, please note that we cannot control the amount of coverage provided, as it is determined by the agreement between you or your employer and the insurance company.
Ultimately, all fees incurred in our office are the patient’s responsibility. We electronically submit all claims to your insurance company and strive to maximize your benefits.
Based on the information you provide about your insurance plan, we estimate your portion of the treatment cost. We ask for this amount on the day of service. Using a credit card guarantee, any remaining balance expected from your insurance company will be held on your account with a 60-day grace period. If the balance remains unpaid by you or the insurance company after this period, it will be transferred to your credit card.

Can I afford cosmetic dentistry?
Cosmetic dentistry offers many different options. Some patients require relatively easy and inexpensive treatments, while others need more extensive procedures to reach their goals. Questions are always answered, and fees are discussed before treatment begins. Out-of-office financing is also available. At Sullivant Dentistry, we need to see that our clients receive the cosmetic dentistry care that they seek, which is why we make financing an option with CareCredit.
Do we accept insurance?
YES, we will gladly submit your insurance after you have met your deductible and co-pay as a courtesy to you and your family. Insurance policies vary in benefits determined by your employer; any remaining balance is your responsibility.
Does Medicare pay on dental?
No, unfortunately dental services are not covered under Medicare.
How much do services cost?
We will gladly give you a cost estimate before work is done; however, in most instances, Dr. Sullivant must see you to determine the best service for your particular need.
